A leading Connecticut medical group is seeking a highly experienced Family Nurse Practitioner or Physician Assistant for a hybrid primary-care role combining remote visits from home with scheduled on-site support. The provider will deliver transitions-of-care visits, annual wellness visits, same-day care, and traditional primary care through a flexible, team-based model.
Job Responsibilities
• Support campus-care teams and deliver traditional primary care virtually during scheduled on-site time
• Carry appropriate clinical responsibility, order and interpret testing, prescribe treatment, and coordinate follow-up
• Maintain efficient visit flow and the demonstrated ability to manage at least 18 patient visits per day when assigned
Qualifications
• At least five years of post-graduate primary-care experience
• Demonstrated success independently carrying a panel and delivering virtual care
• Connecticut licensure or eligibility and a suitable private remote-work environment
