Job Responsibilities
Provide comprehensive medical care for patients in post-acute care and long-term care settings.
Evaluate, diagnose, and manage acute and chronic medical conditions.
Develop, implement, and oversee individualized patient treatment plans.
Collaborate closely with Nurse Practitioners, nursing teams, and interdisciplinary healthcare professionals.
Ensure delivery of safe, effective, and evidence-based patient care.
Monitor patient progress and adjust treatment plans as needed.
Promote quality improvement initiatives and best practices in clinical care.
Maintain accurate, timely, and compliant medical documentation.
Communicate effectively with patients, families, caregivers, and healthcare teams.

Ideal Candidate Qualifications:
Board-certified or board-eligible physician.
MD or DO degree from an accredited medical school.
Specialty background in:
Internal Medicine
Family Medicine
Geriatrics
Related medical specialties
Experience or interest in post-acute care, long-term care, or geriatric medicine preferred.
Strong clinical judgment and decision-making skills.
Excellent communication and interpersonal abilities.
Commitment to delivering compassionate, patient-centered care.
Ability to work effectively within a multidisciplinary healthcare team.
