Client is seeking a full-time Attending Physician (MD/DO) to provide comprehensive medical care to residents of a skilled nursing facility in Granville, New York.
This position offers a collaborative practice model where Advanced Practice Providers (APPs) manage much of the routine daily and acute patient care, allowing physicians to focus on complex clinical decision-making, expand their patient census, increase RVU earning potential, and pursue future leadership opportunities, including Client roles.

Client is seeking a full-time Attending Physician (MD/DO) to provide comprehensive medical care to residents of a skilled nursing facility in Granville, New York.
This position offers a collaborative practice model where Advanced Practice Providers (APPs) manage much of the routine daily and acute patient care, allowing physicians to focus on complex clinical decision-making, expand their patient census, increase RVU earning potential, and pursue future leadership opportunities, including Client roles.
Job Responsibilities
Provide comprehensive medical care for skilled nursing facility residents, including admissions, routine follow-up visits, monthly regulatory visits, and acute evaluations.
Collaborate closely with Nurse Practitioners and Physician Assistants to ensure coordinated, high-quality resident care.
Evaluate, diagnose, and manage acute and chronic medical conditions.
Oversee medication management, including controlled substances and psychotropic medications, while promoting safe prescribing practices and reducing unnecessary polypharmacy.
Lead interdisciplinary care planning meetings and participate in discharge planning to support successful transitions of care.
Provide education and clinical guidance to nursing staff and interdisciplinary care teams.
Support facility compliance during state and federal regulatory surveys and quality initiatives.
Coordinate with local hospitals and specialists to promote continuity of care and reduce unnecessary hospital readmissions.
Maintain timely, accurate, and compliant medical documentation.
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
Active New York State medical license in good standing.
Current DEA registration.
Compliance with all continuing medical education (CME) requirements.
Excellent clinical judgment, communication, and collaboration skills.
Commitment to providing compassionate, patient-centered care for older adults.
Preferred Qualifications:
Previous experience in skilled nursing, long-term care, post-acute care, geriatrics, internal medicine, family medicine, or managed care.
Familiarity with CMS regulations, long-term care documentation, and survey readiness.
Experience working with culturally and linguistically diverse patient populations.
Interest in leadership opportunities and quality improvement initiatives.
