Join our Senior Primary Care Office in Apopka, FL and surrounding areas
Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a family, internal, or geriatric medicine physician to join our growing team as a variable office PRN physician. Grow with us as we expand our reach to help more seniors live better, healthier lives.
You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.
Job Responsibilities
deliver comprehensive outpatient care to adult and senior patients.
maintain accurate medical records and documentation.
will diagnose and manage moderately to complex medical conditions.
will work with interdisciplinary teams to ensure holistic care
Qualifications
-Doctor of Medicine or Osteopathic Medicine
-Board Certified/Board Eligible in family, internal, or geriatric medicine
-Valid, unrestricted license in the FL (or willing to obtain)
-Current, unrestricted DEA license
- Two years' experience seeing patients 65 and over and/or managing patients with chronic medical conditions
Preferred Qualifications:
-Experience managing Medicare Advantage panel of patients with understanding
-Knowledge of Medicare guidelines and coverage
-Knowledge of HEDIS quality indicators
Working Hours
PRN, variable locations
Outpatient only, in a value-based care model.
