Credential with multiple locations at the same time
**building float pool* below are locations that coverage will be needed:
Facility 1 - Mentor, Ohio
Facility 2 - Lorain, Ohio
Facility 3 - Lorain, Ohio
Facility 4 - Willoughby Hills, Ohio
Facility 5 - Brunswick, Ohio
Facility 6 - Fairlawn, Ohio
Outpatient Only
7/1-12/31/2026
Monday - Friday 8a-5p, NO CALL no weekend
minimum 2 weeks per month
Number of patients per day: 15 to 20
Patient Population: Majority is adult primary care patients in ambulatory outpatient practice
Mid-level support: APPs are present within the majority of our practices.
Procedures required: no
