Job Responsibilities
• Collaborate with multidisciplinary care teams during daily clinical huddles and care coordination
• Coordinate specialty referrals, hospital transitions, SNF care, DME, and home health services
• Maintain timely and accurate EMR documentation with coding support assistance
• Participate in quality improvement initiatives and patient outcome optimization
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible Mississippi medical license
• Active DEA license required
• Medicare and Medicaid provider eligibility preferred
