Job Responsibilities
• Collaborate with multidisciplinary care teams during daily clinical huddles
• Coordinate specialty referrals, hospital care transitions, SNF coordination, DME, and home health services
• Maintain accurate and timely EMR documentation with coding support assistance
• Participate in quality initiatives and patient outcome improvement efforts
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible Alabama medical license
• Active DEA license required
• Medicare and Medicaid provider eligibility preferred
Working Hours
