Job Responsibilities
• Collaborate with multidisciplinary care teams in a value-based care environment

• Participate in clinical huddles, quality initiatives, and patient outcome improvement efforts
Qualifications
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible state medical license
• Active DEA license required
• Medicare and Medicaid provider eligibility preferred
Working Hours
