Job Responsibilities
• Conduct annual wellness visits and preventive care screenings
• Develop individualized treatment plans for chronic disease management
• Order and interpret diagnostic testing and laboratory studies
• Provide patient education regarding treatment plans and lifestyle modifications
• Maintain accurate and timely EMR documentation
• Collaborate with specialists and healthcare team members to coordinate care
• Ensure delivery of compassionate, patient-centered healthcare services
Qualifications
• Active Ohio medical license or eligibility to obtain licensure
• DEA certification preferred or required based on state guidelines
• Strong clinical, communication, and interpersonal skills
