A rapidly growing, physician-led healthcare organization is seeking a Primary Care Physician to join a mission-driven outpatient practice serving older adults.

This is a team-based, value-focused model designed to improve outcomes while allowing physicians to practice meaningful medicine with strong clinical support and manageable patient panels.

Position Overview

The PCP provides comprehensive primary care to seniors with acute and chronic conditions, while leading a collaborative care team to drive measurable health outcomes.

This model emphasizes relationship-based care, preventive medicine, and coordinated population health management.

Key Responsibilities

• Provide full-scope outpatient primary care
• Manage complex chronic conditions with a proactive approach
• Lead and coordinate a multidisciplinary care team
• Serve as a health coach, helping patients achieve long-term wellness goals
• Collaborate with specialists and hospitalists for coordinated care
• Participate in quality improvement initiatives and performance meetings
• Maintain accountability for clinical outcomes and patient satisfaction

Qualifications

• MD or DO (Internal Medicine, Family Medicine, Geriatrics or similar specialty)
• Active Missouri medical license (or ability to obtain)
• Board Certified or Board Eligible preferred
• Experience in value-based or managed care environments a plus
• Strong communication and leadership skills
• Comfortable working in a performance-driven, team-based model

Compensation & Benefits

• Competitive base salary approximately $215,000 – $305,000
