Salary: $230,000 – $275,000 (varies depending on years of experience)
About the Opportunity
We’re seeking a compassionate and mission-driven Primary Care Physician to provide comprehensive care in a patient-centered, community-focused setting.
This role is ideal for a physician who values whole-person care, enjoys building long-term patient relationships, and is passionate about supporting individuals with both medical and behavioral health needs.
What You’ll Do
- Conduct comprehensive physical exams and evaluate medical and psychosocial histories
- Diagnose and manage acute and chronic conditions across a diverse patient population
- Prescribe medications and manage ongoing treatment plans
- Order and interpret diagnostic tests, including lab work and screenings
- Identify complex cases and collaborate with physicians or specialists when needed
- Apply evidence-based practices to guide treatment and care planning
- Provide clear, supportive health education to help patients understand diagnoses and treatment options
- Collaborate with interdisciplinary teams to ensure continuity of care
- Support referrals and follow-up care for medical, behavioral health, or social needs
- Adhere to infection control protocols and clinical best practices
- Maintain accurate, timely documentation within EMR systems
Required Qualifications
- MD or DO degree from an accredited institution
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active or eligible for medical licensure in New York
- Minimum of 2 years of clinical experience preferred
- Background in primary care, community health, or addiction medicine
- Experience working with diverse or underserved populations
- Excellent communication and patient engagement abilities
