Seeking BC/BE Family Medicine or Internal Medicine Physician

Overview:
* Join an established nonprofit Primary Care practice serving Southeastern Vermont and Southwestern New Hampshire
* 100% outpatient Primary Care position with an emphasis on comprehensive, patient-centered medicine
* Full-time schedule consisting of 32 patient care hours and 8 administrative hours per week
* Attractive call schedule supporting work-life balance
* Provide comprehensive Primary Care services, including preventive care and chronic disease management
* Evaluate and manage patients with complex medical conditions
* Practice within a certified Patient-Centered Medical Home environment
* Integrated Behavioral Health professionals and Care Coordinators available within the practice
* Access to comprehensive Dental Care, Vision Services and Walk-In Care
* Collaborate with a supportive community health team and multidisciplinary providers
