Job Responsibilities
• Manage acute and chronic conditions
• Perform annual physicals and preventive screenings
• Develop personalized care plans
• Coordinate referrals to specialists when necessary
• Maintain detailed and accurate EMR documentation
• Build long-term patient relationships
Qualifications
• Active South Carolina License (or eligible to obtain)
• 5–20 Years Primary Care Experience
• Stateside U.S. Medical Graduate
