Job description
We are seeking a multi-state licensed Primary Care Physician (PCP) to join our team in a part-time, remote capacity. The ideal candidate plays a key role in screening patients for eligibility to participate in our support programs tailored to individuals with chronic illnesses.
This is a low-time commitment opportunity that provides flexibility while contributing meaningfully to the lives of patients living with long-term health conditions.
Key Responsibilities:
- Screen and assess patients with chronic conditions to determine eligibility for support activities and care coordination.
- Review patient charts and documentation to ensure compliance and appropriateness for program inclusion.
- Work collaboratively with our care team to identify and support patient profiles including those with - Neuropathic pain, COPD, Diabetes, Cancer, Asthma.
Requirements:
- Licensed Primary Care Physician (MD or DO) in good standing.
- Actively enrolled with Medicare.
- Familiarity with chronic disease management and care coordination.
- Experience reviewing medical records and performing eligibility screenings.
- Strong attention to detail and excellent clinical judgment.
- Able to commit 4 hours per week initially, with flexibility to scale up based on program needs.
