Responsibilities:
- Provide comprehensive primary care services to adult patients
- Diagnose and manage acute and chronic conditions
- Emphasize preventive care, wellness, and patient education
- Collaborate with multidisciplinary clinical and support teams
- Maintain timely and accurate medical documentation
Qualifications:
- MD or DO with training in Internal Medicine or Family Medicine
- Board-certified or board-eligible
- Eligible for licensure in New York State
- Commitment to community-based care and patient-centered medicine
