Geriatrics-Focused / Community-Based Care

We are seeking a mission-driven MD/DO physician trained in Internal Medicine or Family Medicine to provide comprehensive primary care to a medically complex senior population. This role is ideal for a physician interested in geriatrics, care coordination, and relationship-based medicine outside of a high-volume primary care office setting.

The physician will serve as a primary care provider within a multidisciplinary model designed to support older adults who qualify for nursing home–level care but remain living in the community.

Practice Setting

  • Community-based senior care model
  • Patients are seen primarily at a central care site, with occasional home or assisted living visits
  • Slower pace than traditional outpatient primary care
  • Focus on complex, high-acuity geriatric patients
  • Strong emphasis on patient and family education

Key Responsibilities

  • Deliver longitudinal primary care to enrolled senior patients in community and residential settings
  • Perform comprehensive history and physical exams for new patients and routine reassessments for established patients
  • Evaluate and manage acute medical issues and chronic conditions
  • Participate actively in interdisciplinary team meetings, including care planning and case conferences
  • Collaborate closely with nurses, nurse practitioners, specialists, and social services to ensure continuity of care
  • Communicate with families and caregivers regarding patient condition and care plans
  • Coordinate transitions of care, including post-hospital follow-up and discharge planning
  • Monitor patients hospitalized outside the network and assist with care coordination when appropriate
  • Maintain timely and accurate documentation in the electronic medical record
  • Review consultant recommendations and implement care changes promptly
  • Participate in quality, utilization, and regulatory meetings as needed
  • Provide clinical oversight and mentorship to advanced practice providers
  • Participate in an after-hours call rotation (phone-based only)

Schedule & Workload

  • Monday–Friday schedule
  • Standard business hours (no overnight shifts)
  • Average ~10 patients per day
  • Mix of comprehensive assessments and acute visits
  • Occasional home or assisted living visits (approximately 2–3 times per month)

Candidate Qualifications

  • MD or DO required
  • Completion of residency in Internal Medicine or Family Medicine
  • Board Certified or Board Eligible
  • Geriatric experience strongly preferred (training available for the right candidate)
  • Comfortable caring for medically complex, frail, elderly patients
  • Strong communication and interpersonal skills
  • Team-oriented mindset with the ability to collaborate across multiple disciplines
  • Mission-driven approach to patient care
  • Ability to travel locally within the Bronx and surrounding areas
  • Bilingual skills are a plus but not required
  • Clean professional background
