Annual Salary: $180k-$215k (varies depending on years of experience)

We are seeking a dedicated Primary Care Physician to provide high-quality, patient-centered medical care in an outpatient clinical environment. This role delivers comprehensive, coordinated primary care services while actively contributing to clinical quality improvement, team development, and overall program success.

Key Responsibilities

  • Deliver comprehensive primary care services in an ambulatory setting using evidence-based medical practices.
  • Develop, implement, and manage individualized treatment plans for a diverse patient population.
  • Provide case-managed medical care in collaboration with interdisciplinary clinical and support teams.
  • Participate in clinical quality improvement initiatives and performance monitoring activities.
  • Contribute to staff education, training, and professional development programs.
  • Maintain thorough, timely, and accurate medical documentation in accordance with regulatory and organizational standards.
  • Remain current on best practices, clinical guidelines, and advancements in medical care.
  • Support public health initiatives and preventive care efforts.
  • Promote continuity of care through appropriate follow-up and care coordination.
  • Participate in additional clinical and administrative responsibilities as assigned.

Required Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
  • Successful completion of an accredited residency program.
  • Active and unrestricted New York State medical license.
  • Board Certified or Board Eligible.
  • Strong clinical knowledge across general medical practice.
  • Demonstrated ability to design and manage comprehensive treatment plans.
  • Prior experience in public health or community medicine preferred.
  • Bilingual Spanish skills strongly preferred.
