Job description

Job description The Family Practice Physician is responsible for providing high-quality, comprehensive primary care to patients of all ages in a community-based, Federally Qualified Health Center (FQHC). This position focuses on delivering preventive care, chronic disease management, and acute medical services with a strong emphasis on reducing health disparities and improving outcomes in underserved populations. The physician works collaboratively with a multidisciplinary team to ensure continuity and coordination of care.

Job Responsibilities

Provide direct primary medical care to adult patients (ages 18+), including preventive, acute, and chronic care management.

Perform comprehensive physical exams, health risk assessments, and age-appropriate screenings.

Diagnose and treat a broad range of internal medicine conditions, including hypertension, diabetes, cardiovascular disease, and other chronic illnesses.

Order and interpret diagnostic tests and lab results to guide clinical decision-making.

Prescribe medications and develop individualized treatment plans based on current clinical guidelines.

Document all clinical encounters accurately and promptly in the Electronic Health Record (EHR).

Collaborate with a multidisciplinary team including behavioral health, dental, case management, and specialty providers.

Participate in quality improvement initiatives and adhere to standards established by HRSA, NCQA-PCMH, and other regulatory agencies.

Qualifications

Licensure/Certification:

Unrestricted medical license in Georgia

Board-certified or board-eligible in Internal Medicine

DEA license with appropriate schedule authority

Current BLS/CPR certification

Experience in a community health, FQHC, or underserved setting preferred

New graduates with strong interest in community health will be considered

Ability to work with diverse patient populations

Proficient in use of Electronic Health Records
