Job description
We are seeking a dedicated and compassionate Primary Care Physician to join our team in providing exceptional care to patients in skilled nursing and long-term care facilities. In this role, you will be responsible for rounding and seeing patients for regulatory and follow-up visits, ensuring that each patient receives the highest standard of care. This position offers a structured Monday through Friday schedule with no on-call responsibilities, as we have a dedicated on-call team that handles after-hours and weekend care.
Job Responsibilities
Patient volume expectations are 15 per day
Perform regulatory visits, including admission, re-certification, and follow-up visits
Manage chronic conditions and coordinate care plans for patients
Collaborate with the multidisciplinary team to optimize patient outcomes
Maintain accurate and up-to-date patient records
Ensure compliance with facility policies and healthcare regulations
Work closely with nursing staff, social workers, and other healthcare providers to ensure comprehensive patient care
Provide support for patients and families, addressing questions and concerns as necessary
Qualifications
Board Certification or Eligibility in Internal Medicine, Family Medicine, or a related field
Active and unrestricted medical license to practice in Colorado
Strong knowledge of regulatory requirements in skilled nursing and long-term care settings
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
