What You’ll Do

Lead clinical case reviews and ensure high standards of patient care

Mentor and provide oversight to advanced practice clinicians across multiple facilities

Collaborate with pharmacists, nurses, and leadership on medication management, care coordination, and quality initiatives

Build strong community relationships and represent the practice with professionalism

Develop and implement individualized patient care plans

Provide timely charting and response to pharmacy recommendations
What We’re Looking For

Board-certified physician (FM, IM, EM, or related discipline)

Background in post-acute, skilled nursing, home health, hospice, palliative, or hospital medicine preferred

Prior medical director experience is a plus but not required
Compensation & Benefits

Fee-for-service structure; annual earnings typically range from $310,000–$345,000+ with uncapped productivity potential
