Meridian Health Partners is currently looking for both part-time and full-time physicians to join our growing team in post-acute/skilled nursing facilities and residential senior living communities. We have immediate needs in the Fort Worth, Glen Rose and Weatherford areas.

Job Responsibilities

Core Responsibilities:
Provide in-person medical care to patients in assigned post-acute facilities
Perform assessments, manage treatment plans, and update documentation in the EMR
Lead or collaborate with facility teams to ensure quality outcomes and smooth transitions of care
Develop strong working relationships with facility staff and leadership
Be available in assigned facilities anytime during regular business hours on scheduled days

Qualifications

Qualifications:
MD or DO with a current, unrestricted Texas medical license
Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
1+ year of experience in post-acute, geriatric, or SNF care preferred
Comfortable practicing independently in skilled nursing facilities
Strong clinical judgment, communication skills, and a patient-centered mindset
