- Not accepting Locums candidates
- Not accepting visa candidates
- Cannot accept providers who have any malpractice cases that are in Probate or that require supervision. Clean malpractice is preferred.
- ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
- Board Eligible (within 3 years of residency) or Board Certified in Family Medicine or Internal Medicine - REQUIRED
- IL license (or willing to obtain) - REQUIRED
- Clean Malpractice - PREFERRED
- Board Eligible (within 3 years of residency) or Board Certified in Family Medicine or Internal Medicine - REQUIRED
