Job Description

A Moreno Valley based client is seeking an Internal Medicine physician for providing preventative, curative, and rehabilitative medical care to assigned patients and determining appropriate regimen of health care for each patient. Maintaining supervision of case management of patients including but not limited to: referral to appropriate specialists, ancillary services, arranging home care and hospital care if needed, call backs to pharmacies and patients as needed.

Required Qualifications

  • Attended an accredited medical school with a MD or DO degree
  • Completed an accredited residency program
  • DEA certificate
  • Board certification or eligible in internal medicine
  • BLS certification

Responsibilities

  • Plan and provide the care for and the management of stabilized chronic conditions, asymptomatic and well patients of all ages and socioeconomic cultures.
  • If asked, assist in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services.
  • Assists in maintaining and implementing protocols for Quality Assurance on a routine basis.
  • Will serve, as needed, as preceptor for medical students.
  • Initiate referrals to medical specialties for assigned patients as needed.
  • Responsible for initiating referral to the emergency room/hospital or extended care facilities for assigned patients as needed.
  • If asked, assist the Quality Team in implementing and maintaining quality procedures and protocols.
  • Assist and educate other/new Providers in complying with Policies and Procedures
  • Maintain professional and excellent working relationship and dialogue with Site Manager/Supervisor, Registered Nurse(s), and other providers to assure smooth operation of clinic.
  • Responsible for participating in quality improvement projects/activities and accountable for quality.
  • Every attempt should be made to solve all problems at clinic level.
  • Will report direct patient care issues to the Medical Director and any other issue(s) (i.e. administrative) to the Site Manager.
  • Will be required to attend Provider meetings and various clinic or corporate meetings, training or seminars.
  • Based on business needs, you may be required to provide coverage at other CHSI clinic locations other than your home site.
  • May be required to work weekends or evenings as designated.
  • Perform all other duties as directed either formally or informally, verbally or in writing.
