A healthcare network in New York City is currently seeking a licensed Nurse Practitioner (NP) to join their growing team in Manhattan.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: Standard business
  • Specialty: ISNP / Managed Care
  • Setting: Case Management Department
  • Patient Population: Geriatric

Responsibilities:

The Nurse Practitioner (NP) will:

  • Provide and/or coordinate primary clinical care services for assigned high risk members and facilitate supportive care during transitions from acute or sub-acute care setting.
  • Collaborate with Plan Case Manager to schedule and follow-up home visits to update the care plan
  • Ensure safe transition from setting to setting, conducts post discharge assessment within a reasonable time period
  • Evaluate appropriateness of member transfers for acute inpatient services, prospectively and retrospectively and share necessary clinical information with collaborating physicians/facilities
  • Provide individualized care for the at risk population that would improve care coordination, reduce avoidable utilization of hospitals, improve the health and quality of life of community dwelling members
  • Perform other duties, as needed

Qualifications:

  • Bachelor's and/or Master's Degree
  • NYS Nurse Practitioner (NP) license

Desired Skills:

  • Previous experience in an Acute, Subacute, and/or Long Term Care setting
  • Managed Care background
  • ISNP or Medicare Advantage experience
