A healthcare network in New York City is currently seeking a licensed Nurse Practitioner (NP) to join their growing team in Manhattan.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: Standard business
- Specialty: ISNP / Managed Care
- Setting: Case Management Department
- Patient Population: Geriatric
Responsibilities:
The Nurse Practitioner (NP) will:
- Provide and/or coordinate primary clinical care services for assigned high risk members and facilitate supportive care during transitions from acute or sub-acute care setting.
- Collaborate with Plan Case Manager to schedule and follow-up home visits to update the care plan
- Ensure safe transition from setting to setting, conducts post discharge assessment within a reasonable time period
- Evaluate appropriateness of member transfers for acute inpatient services, prospectively and retrospectively and share necessary clinical information with collaborating physicians/facilities
- Provide individualized care for the at risk population that would improve care coordination, reduce avoidable utilization of hospitals, improve the health and quality of life of community dwelling members
- Perform other duties, as needed
Qualifications:
- Bachelor's and/or Master's Degree
- NYS Nurse Practitioner (NP) license
Desired Skills:
- Previous experience in an Acute, Subacute, and/or Long Term Care setting
- Managed Care background
- ISNP or Medicare Advantage experience
