Job description
- Provide comprehensive, patient-centered primary care to assisted living residents, managing acute and chronic conditions while ensuring quality care, continuity, and timely follow-up.
• Salary: $200,000–$230,000 per year
• Employment Type: Full-Time (W-2 or 1099 options may be available)
Job Responsibilities
• Conduct routine examinations, wellness visits, and preventive screenings.
• Prescribe medications and coordinate referrals and follow-up care.
• Collaborate with nursing teams, facility staff, residents, and families.
• Maintain accurate medical records and ensure timely documentation.
• Respond promptly to clinical concerns and support continuity of care.
Qualifications
• Active, unrestricted New York State medical license.
• Board certified or board eligible in the applicable specialty.
• Active DEA registration and Medicare/Medicaid enrollment.
• Minimum 3 years of experience.
• Geriatric or long-term care experience preferred.
Working Hours
