Build lasting patient relationships in this full-time outpatient opportunity in the Jacksonville area. You'll care for older adults, helping them manage chronic conditions and take a proactive approach to their health. An integrated team helps with patient services, referrals, and follow-up, and this day-shift clinic role includes 40 hours per week with time for administrative work.
