Job description
Bluegrass Care Navigators is seeking a Geriatric Medicine Physician to join our interdisciplinary care team in Lexington, Kentucky. This physician will provide clinical care, geriatric consultation, and medical expertise across BCN programs, with a primary focus on patients receiving care in skilled nursing facilities.
This is an opportunity to work alongside an experienced interdisciplinary team while providing thoughtful, patient-centered care to older adults with complex medical and chronic care needs. The physician will also serve as a clinical resource to BCN providers and interdisciplinary team members, contributing to education, mentorship, quality initiatives, and collaborative care planning.

Job Responsibilities
Provide primary medical care and attending physician support to patients within designated service lines, particularly skilled nursing facilities
Serve as a clinical resource for patients receiving care in skilled nursing facilities
Collaborate and communicate with patients’ attending physicians and other healthcare professionals
Participate in interdisciplinary rounds and team meetings
Provide education, training, mentorship, and clinical guidance to BCN providers and team members
Participate in triage and cross-coverage as needed within area of clinical expertise
Support quality, safety, pharmacy management, and ethical initiatives
Participate in physician on-call coverage, including evenings, weekends, and holidays, according to departmental needs
Maintain an active DEA license for controlled-substance prescribing
Participate in ongoing professional development and organizational education requirements
Qualifications
Board certified or board eligible in Family Medicine, Internal Medicine, or another appropriate specialty
Geriatric Medicine certification preferred
Certification or eligibility for Certified Medical Director (AMDA) preferred
Hospice and Palliative Medicine experience or certification preferred
Two or more years of clinical practice experience preferred
Strong knowledge and clinical experience in geriatric medicine
Experience with complex chronic disease and pain and symptom management preferred
Excellent communication and collaboration skills
Ability to work effectively as part of a large interdisciplinary team
Ability to provide clinical consultation, education, and mentorship
Familiarity with quality management and ethical considerations in geriatric, hospice, and palliative care
Valid driver's license, reliable transportation, and automobile insurance meeting state requirements
