Job description
Conduct virtual clinical assessments for Medicare beneficiaries (aged 65+) and design Chronic Care Management (CCM) plans executed by a dedicated support team. Enjoy 100% remote flexibility, complete clinical autonomy, and zero administrative burden. Please note, this position does require multi-state licensure of 40+ states.
Job Responsibilities
Care Strategy & Leadership: Review pre-charted patient data, synthesize complex medical histories/medications, and direct non-clinical care managers and social workers who execute long-term follow-ups.
Long-Term Oversight: Maintain visibility into patient progress, review assigned caseloads (1–2 hours/week), and conduct formal patient reassessments every 6 to 12 months.
EHR Documentation: Maintain accurate, timely clinical notes and time logging in Canvas EHR.
Patient Engagement: Build trust with elderly patients and their families via video while maintaining strict adherence to scheduled appointment times.
Qualifications
Licensure: Active 40+ state medical license or active multi-state licenses with willingness to obtain additional state licensures as coverage expands.
Clinical Expertise: Deep background in managing complex chronic conditions, polypharmacy, dementia care, fall prevention, and value-based/CCM care models.
Technical Setup (BYOE): Personal computer setup with high-speed internet, webcam, and hardware capable of running Canvas EHR and Zoom simultaneously.
Working Hours
Operating Window: Core operating hours are Monday – Friday, 11:00 AM – 4:00 PM EST/PST, but you can build your own schedule (weekend flexibility available if preferred).
