Job Responsibilities
Develop long-term relationships with patients and their families
Manage chronic conditions including:
Diabetes
Hypertension
COPD
Congestive Heart Failure
Chronic Kidney Disease
Deliver preventive care and wellness-focused treatment plans
Coordinate care with multidisciplinary teams including APPs, pharmacists, and care coordinators
Utilize EMR systems, clinical data, and quality tools to support improved patient outcomes
Participate in quality improvement initiatives and evidence-based care programs
Maintain accurate clinical documentation and regulatory compliance
Qualifications
Board Certified or Board Eligible in:
Family Medicine
Internal Medicine
Geriatrics
Active medical license or ability to obtain state licensure
Minimum 3 years of outpatient primary care experience preferred
Strong background managing complex chronic conditions
Passion for senior care and relationship-based medicine
Excellent communication and interpersonal skills
Commitment to quality-focused, value-based healthcare
Preferred:
Medicare Advantage experience
Population health experience
Value-based care experience
