Medical Director – Remote
Location: Fresno, CA (Must hold active CA license)
Position Overview
We are seeking a Board-Certified Medical Director to provide both clinical leadership and direct patient care for a high-acuity patient population. This is a fully remote role with no nights, weekends, or on-call responsibilities, offering an exceptional work-life balance. The position combines strategic clinical oversight with hands-on care focused on keeping high-risk patients safely out of the hospital.

70% Clinical Leadership

30% Direct Patient Care

Approximately 2 days patient care / 3 days leadership per week

Key Responsibilities
-
Provide clinical leadership, oversight, and guidance to interdisciplinary care teams

Support quality initiatives, care model development, and clinical best practices

Deliver direct patient care including comprehensive head-to-toe assessments in patient homes

Manage and care for a medically complex, high-risk patient population
-
Collaborate with care teams to prevent avoidable hospitalizations and improve outcomes

Ensure high standards of clinical quality, safety, and patient-centered care
Required Qualifications

Active, unrestricted California medical license

Board Certification in one of the following:

Family Medicine
-
Internal Medicine
-
Emergency Medicine
-
Geriatric Medicine
-

Experience caring for complex or high-risk patient populations

Strong leadership, communication, and clinical decision-making skills
