We’re looking for a dedicated and empathetic Nurse Practitioner (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner) to join a collaborative team serving residents in long-term care and skilled nursing settings.

This is a meaningful opportunity for a provider who wants to build lasting relationships with patients while working in an environment that values balance, teamwork, and professional growth.

What You’ll Do:

  • Maintain clear, timely documentation and communicate effectively with facility staff and family members

What We’re Looking For:

  • Active Nurse Practitioner license (APRN) — FNP or AGNP

  • Experience in geriatrics, long-term care, or primary care preferred (new graduates encouraged to apply)

  • Commitment to patient-centered care and strong communication skills

  • Ability to work independently and as part of a supportive care team

What’s Offered:

  • Salary range: $100,000 – $120,000

  • Comprehensive medical, dental, and vision insurance

  • 401(k) with employer contribution

  • Paid Time Off (PTO) and paid holidays

  • Malpractice coverage provided

  • Consistent weekday schedule for true work-life balance

  • Ongoing training and professional development opportunities
