CU/Critical Care JOB DETAILS: ___________________________________________________
We are looking for a provider who can help with 7 shifts a month (7am-7pm) of ICU coverage. Please ensure provider bids reflect the below: - Guaranteed Hours: 12 hours per day There is no OT expected for this position. If you must enter OT application in, please ensure it reflects after 12 hours. REQUIREMENTS: ___________________________________________________
Active CA License and CA DEA Board Certified or Board Eligible provider within 3-5 years of residency BLS and ACLS certifications
PRIVILEGING: _________________________________________________________________
Please note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days. o Typical privileging timeline is 90 days o Once the file is completed, we can look at temporary privilege eligibility. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
TRAVEL REQUIREMENTS: _____________________________________________________
Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return. WILL be reimbursed: o Airport parking at provider’s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
