Requirements
· Board Certification: BC Only
· License required: Active WA or IMLC
· DEA: Required
· Fellowship: Preferred
Credentialing Timeframe: 90 Days

We are looking to add one locum to our locum pool who can cover all of the following dates: Sept 8-21 (6am-6pm) Oct 9-12 (starting 6pm and finishing by 6am on Oct 13), Dec 22nd to Dec 28th (Starting 6pm, finishing at 6am on Dec 29th), Dec 22nd to Dec 28th (6am-6pm)
