Hospice Physician Opportunity
Located in Albany, NY – Serving New York's Capital Region
Part-time, permanent position
Monday–Friday, every other week
Position Overview
Part-time Hospice Physician opportunity within an established community hospice program
Provide compassionate, patient-centered end-of-life care across home, skilled nursing facility, and hospital settings
Monday–Friday schedule, working every other week
Collaborate with an interdisciplinary hospice team to develop individualized care plans
Share on-call responsibilities with a team of 5 Hospice Physicians
Serve patients across 8 counties in New York's Capital Region

Physician qualifications and board certification requirements not specified
Experience or interest in Hospice and Palliative Medicine
Strong communication and interdisciplinary collaboration skills
Commitment to compassionate, patient-centered end-of-life care
Practice Highlights
Established community hospice program serving 8 counties
Majority of patient care delivered in patients' homes
Comprehensive hospice services across inpatient, outpatient, and home-based settings
Collaborative interdisciplinary team, including:
- Hospice Physicians
- Nurses
- Social Workers
- Chaplains
Clinical responsibilities include:
- Physician oversight and medical management of hospice patients
- Development and supervision of individualized care plans
- Initial and ongoing hospice eligibility certification
- Determination of appropriate hospice levels of care
- Medication management and prescribing
- Participation in interdisciplinary team meetings and care conferences
- Clinical consultation with community healthcare providers
Teaching opportunities involving medical students, NP/PA students, residents, and Geriatrics Fellows
Shared call coverage among 5 Hospice Physicians

Compensation: $140,000
