Position Highlights

• Part-Time position (20 hours per week/O.5 FTE) with the potential for on-site work at various facilities.
• Engage in weekly Interdisciplinary Group (IDG) meetings, providing clinical leadership and expertise.
• Certify and recertify patient eligibility for hospice care, ensuring comprehensive and accurate documentation.
• Participate in developing and revising comprehensive plans of care for each patient.
• Equitable share of on-call coverage, including overnight and weekend responsibilities.

• Competitive salary range of $130,000 - $140,400, based on experience.
