We are seeking a Hospice & Palliative Medicine Physician to join our team in Louisville and provide thoughtful, patient-centered care across inpatient and community settings.

The Opportunity

• Provide attending and consulting physician services for hospice and palliative care patients.

• Collaborate with interdisciplinary teams, community physicians, nurse practitioners, and families to coordinate individualized care.

• Conduct medically necessary patient visits, consultations, and hospice certifications and recertifications.

• Participate in rotating inpatient and organization-wide night and weekend call coverage.

• Contribute to physician education, community outreach, clinical quality initiatives, and the development of strong relationships with healthcare partners.

Qualifications

• MD or DO required.

• Kentucky and Indiana medical licensure, or eligibility to obtain both, including applicable DEA registrations and controlled substance permits.

• Fellowship training in Hospice and Palliative Medicine or at least two years of post-residency medical experience.

• Board certification or board eligibility, with certification expected within five years of employment.

• Experience in hospice, palliative care, primary care, hospital medicine, or geriatrics preferred.

• Hospice and Palliative Medicine certification preferred.
