NP - Hospice & Palliative Care– Ongoing Locum Tenens - Escondido, California - (DM)

Start Date: September 28, 2026
Assignment: Ongoing | 13 weeks with potential to extend
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Weekend: One Sunday per month, with a weekday off during that week
Call: Optional / Occasional On-Call Coverage
Work Environment: Home-Based Hospice & Palliative Care
Patient Volume: Approximately 45–50 visits/month
EMR: MyUnity
Local Travel: Required for home visits

Clinical Responsibilities

• Conduct face-to-face hospice and palliative care visits in patients' homes
• Perform comprehensive assessments and manage symptoms
• Develop and update individualized care plans
• Participate in goals-of-care discussions and advance care planning
• Assist with POLST and advance directive discussions
• Provide education and support to patients and families
• Coordinate care with physicians, community providers, and the interdisciplinary team
• Participate in interdisciplinary team meetings and quality initiatives
• Maintain timely and accurate clinical documentation

Requirements

• Active California NP and RN licenses
• DEA registration
• NPI
• MSN degree
• AANP or ANCC certification
• BLS certification
• 3+ years of hospice or palliative care experience
• ACHPN certification preferred at hire and required within one year
• Valid California driver's license and appropriate vehicle insurance
• Comfortable providing independent home-based care
• Spanish bilingual skills are a plus
