Job Responsibilities
• Lead goals-of-care discussions and advance care planning with patients and families
• Coordinate care across multidisciplinary teams including primary care and specialty providers
• Maintain accurate, timely, and compliant medical documentation

Qualifications
• Active Oregon Medical License (or ability to obtain)
• DEA registration and current CPR certification
• Experience in palliative care, hospice, or supportive care settings preferred
