A growing care organization is seeking a compassionate physician to join its palliative services team. This role centers on supporting individuals with advanced illness through thoughtful clinical management, collaborative care planning, and clear communication with families and referring partners.

Key Responsibilities

  • Participate in determining and re-evaluating patient eligibility for palliative or end-of-life services, ensuring accurate certification of prognosis throughout the course of care.
  • Partner with interdisciplinary care teams to shape, update, and execute individualized medical plans, contributing clinical insight and supporting holistic care goals.
  • Provide expert guidance to admissions and clinical staff on complex cases involving symptom management, disease progression, and determining appropriateness of hospice or palliative services.
  • Review and monitor medication strategies, adjusting regimens to optimize comfort, minimize side effects, and ensure clinical appropriateness.
  • Offer direct medical oversight when an attending physician is unavailable or unable to manage aspects of care, which may include prescribing, care coordination, or completion of end-of-life documentation.
  • Conduct home-based assessments, as needed, based on patient need and care plans developed with clinical leadership.
  • Document all clinical encounters and recommendations in accordance with regulatory requirements and internal policy.
  • Support families by offering clear, empathetic explanations regarding prognosis, treatment goals, and evolving care needs.
  • Coordinate with hospitalists, attendings, and community physicians to ensure seamless transitions between care environments.
  • Address issues related to potential or actual revocation of hospice or palliative services, working closely with team members to clarify concerns and determine next steps.
  • Contribute to education and outreach efforts with community partners to improve awareness of palliative options and strengthen referral pathways.
  • Assess referrals—particularly those with oncologic conditions—to determine eligibility and recommend the most appropriate level of support.
  • Provide clinical leadership to internal and external partners to enhance continuity, quality, and accessibility of palliative care services.

Qualifications

  • Medical degree (MD or DO) from an accredited institution.
  • Board Certification or Board Eligibility in Hospice & Palliative Medicine, Internal Medicine, Family Medicine, Geriatrics, or a closely related specialty.
  • Active, unrestricted medical license in New York.
  • Prior experience in palliative care, hospice, or complex chronic disease management is preferred.
  • Skilled in leading interdisciplinary teams with clarity, empathy, and clinical integrity.
  • Excellent communication skills, particularly in navigating sensitive discussions with patients and families.
