• MD or DO
• Completion of an accredited Internal Medicine residency
• Completion of a Hematology/Oncology fellowship
• Board Certification in Internal Medicine and Hematology/Oncology, or ability to obtain required certification within 3 years of employment
• Current or eligible Indiana medical license
• Ability to obtain hospital privileges
• Valid controlled substance registration
• DEA registration
