Position Overview

The Rural Health Hub (RHH) is seeking a Tele-Hematologist/Oncologist to provide high-quality specialty care to adults with hematologic and oncologic conditions in rural and medically underserved Texas communities.

This flexible, part-time role allows the hematologist/oncologist to work remotely while trained clinical staff are physically present with the patient. The physician will evaluate and manage a broad range of benign and malignant hematologic conditions, provide oncology follow-up and survivorship care, and collaborate with local primary care providers, hospitals, laboratories, infusion centers, surgeons, radiation oncologists, and regional cancer centers.

The Rural Health Hub Model

RHH uses a hybrid-telehealth model that combines remote specialty expertise with in-person clinical support.

Local medical assistants, nurses, or advanced practice providers assist with:

• History, medications, vital signs, and symptom assessment

• Physical examination components

• Assessment for pallor, bruising, bleeding, lymphadenopathy, edema, and other relevant findings

• Connected examination technology when appropriate

• Coordination of CBCs, coagulation studies, chemistry panels, and other laboratory testing

• Coordination of imaging, pathology review, and other diagnostic studies

• Medication reconciliation and treatment monitoring

• Referrals, follow-up care, and communication with the local and regional care teams

This model allows hematologist/oncologists to provide meaningful longitudinal specialty care while using local and regional resources for laboratory testing, imaging, transfusions, infusions, procedures, chemotherapy, radiation therapy, hospitalization, and higher levels of care when needed.

Key Responsibilities

• Conduct initial and follow-up hematology and oncology visits by telehealth

• Evaluate and manage common benign and malignant hematologic conditions

• Provide oncology follow-up and surveillance when clinically appropriate

• Develop individualized diagnostic and treatment plans

• Review laboratory results, pathology, imaging, molecular testing, prior treatment records, and clinical progress

• Order appropriate diagnostic testing and referrals

• Provide longitudinal medication and disease management

• Monitor patients with chronic hematologic disorders

• Provide cancer survivorship and post-treatment follow-up

• Support symptom management and treatment-related complication monitoring

• Educate and collaborate with patients, families, and caregivers

• Identify patients requiring urgent, procedural, or in-person evaluation

• Coordinate care with primary care clinicians, hospitals, surgeons, radiation oncologists, infusion centers, palliative care teams, and other specialists

• Complete timely documentation and clinical follow-up

Clinical Scope

Patients may include adults with:

• Iron deficiency anemia and other anemias

• Thrombocytopenia

• Neutropenia and leukopenia

• Polycythemia and erythrocytosis

• Leukocytosis

• Hemolytic anemia

• Sickle cell disease

• Thalassemia

• Hemophilia

• Von Willebrand disease

• Bleeding disorders

• Thrombophilia and clotting disorders

• Venous thromboembolism

• Monoclonal gammopathy of undetermined significance (MGUS)

• Myeloproliferative disorders

• Myelodysplastic syndromes

• Chronic leukemias

• Lymphoma

• Multiple myeloma

• Breast, lung, colorectal, prostate, and other solid tumors

• Cancer treatment follow-up and surveillance

• Treatment-related cytopenias and complications

• Cancer survivorship

• Late effects of cancer therapy

• Abnormal blood counts requiring specialty evaluation

• Abnormal pathology, imaging, or laboratory findings requiring hematology/oncology evaluation

Patients requiring bone marrow biopsy, chemotherapy, immunotherapy, transfusion, radiation therapy, surgery, stem cell transplantation, or other advanced oncologic treatment would be coordinated with appropriate local or regional treatment centers and hospital-based resources.

Qualifications

• MD or DO degree

• Board certified or board eligible in Hematology and/or Medical Oncology

• Active Texas medical license or eligibility to obtain one

• Strong hematology and oncology diagnostic and management skills

• Ability to practice effectively through telehealth

• Strong communication skills with patients and families

• Strong collaboration skills with local clinicians and tertiary care teams

• Commitment to rural and underserved populations

Experience in telemedicine, rural healthcare, benign hematology, cancer survivorship, chronic hematologic disorders, solid tumor oncology, or oncology follow-up is welcomed but not required.
