Locums Hematology Oncology need in IA

Schedule:

• Call Rotation: Night Call Monday-Friday (after being onsite for Clinic during the day) and 24 Hour Call the weekend before or after their scheduled week
• Call is required anytime they are scheduled to be working Monday-Friday
• Patient Contact Hours: 6
• Clinic Days: 5
• Clinic Hours: 8a-5p
• Procedure/OR Days: 0
• On Call: Dependent on Locums/Part Time Providers Number
• Coverage will only be inpatient. There is not an outpatient component right now.

Job Responsibilities:

Bill Rates Needed for Submission:

Orientation $200/hr

Hourly = $600/hr

Holiday Hourly = $700/hr

Night Call = $500 (to include 3 gratis hours)

Holiday Night Call = $700 (to include 3 gratis hours)

24 Hour On Call= $4000 (to include 6 gratis hours)

Holiday 24 Hour On Call = $5000 (to include 6 gratis hours)

Call Back = $700/hr

Travel Reimbursement:

• Flights - $750/round trip cap (to include all taxes and fees). No upgraded airfares or seat upgrades
• Lodging - 1.5x the GSA rate cap average for the total stay (to include taxes and fees)
• Rental Car - $55/day cap (including all taxes and fees) regardless of car class. SUV/upgrades over cap will need to be preapproved (will only be approved if needed for inclement weather)
• Mileage - IRS Standard Rate

Submission Requirements

• Updated CV (No gaps above 30 days)
• NPDB self-query ( Within 30 days of presentation)
• Board Certified or Truly Board Eligible in Hematology and Oncology - [PREFERRED]
• Willing to review candidates Board Certified in Oncology with Hem/Onc experience & willing to see Hematology patients - please note at time of name clear - [REQUIRED]
• Active IA License or Active IMLC (please upload LOQ at time of name clear) - [REQUIRED]
• Fellowship Trained in Hematology & Oncology - [REQUIRED]
• Must be experienced - [REQUIRED]
