Contract Details
- Location: Anchorage, AK
- Practice Setting: Hospital
- Start Date: As soon as credentialed
- Shift/Schedule Details: 4, Day (10 Hours) 7:30 - 16:30
- Credentialing Timeframe: 90 days
- General Requirements:
- Board Certification: BC
- License Required: AK
- Volume/# of patients per day- 12-13 patients per day 1hr new consults 30 mins follow ups
- Support Staff: 3 FTE MD’s, 1 FTE NP, 1 per diem NP
