Weekend Requirements: Yes
On Call Requirements: 1:3 -evening every night//weekend Friday 4pm-Mon 7AM
Board Certified - [REQUIRED]
CT license in hand at the time of submission - NO exceptions - [REQUIRED]
BLS - [REQUIRED]
Provider Must be local - only mileage is reimbursed
A Self-Query NPDB report is required at the time of presentation and must be pulled within the same month as the submission. - [REQUIRED]
