Contract Details
- Location: Prestonsburg, KY
- Practice Setting: Clinic/Private Practice: outpatient
- Start Date: As soon as credentialed
- Shift/Schedule Details: 5, Day (8 Hours) 8:00 - 5:00
- Credentialing Timeframe: Approx. 60 days
- General Requirements:
- Board Certification: BC in Hematology & Oncology
- License Required: KY
- Support Staff: one APP on-site daily. Dedicated oncology-trained RNs support infusion services during clinic hours.
- Patients per day: ~15/day, low volume, solo provider
