Overview
A healthcare facility in Plymouth, IN is seeking a Board-Certified Hematology/Oncology Physician for a locum-to-perm opportunity. This role focuses on outpatient infusion coverage across multiple locations with a stable patient base and strong clinical support.
🩺 Position: Physician – Hematology Oncology
📍 Location: Plymouth, IN (with coverage at Mishawaka campus)
🏥 Practice Setting: Outpatient Oncology / Infusion Centers
Opportunity Highlights
• Locum-to-Perm opportunity supporting a new service line
• Coverage split: 3 days in Mishawaka / 2 days in Plymouth
• Evening call coverage required
• 30 patients per day (outpatient average)
• 0 inpatient rounding responsibilities
• Patient population: Adult & Geriatric
• Practice mix: 50% Hematology / 50% Oncology
• Work in a structured outpatient oncology setting
• EPIC EMR system
Facility Details
• Outpatient oncology and infusion-based practice
• Credentialing timeframe:
• ~60 days with IN license
• ~90 days without IN license
Required Procedures & Duties:
• Management of VAD/CV catheters
• Bone marrow biopsy
• Participation in tumor board
• Management of bone marrow transplants
• Order tests, imaging, and blood bank services
• Prescribe and manage oncology treatments
• Administration of chemotherapy (with nurse oncologist support)
Candidate Requirements
✔ Board Certified (ABMS/AOA in one specialty with competency in the other) – REQUIRED
✔ Active Indiana License or IMLC with LOQ – REQUIRED
✔ BLS Certification – REQUIRED
✔ Commitment to minimum 6-month assignment – REQUIRED
✔ NPDB Self-Query report within 30 days – REQUIRED
Credentialing Documentation Required
Providers should have the following ready to expedite credentialing:
• Medical diploma
• Residency / fellowship certificates
• All medical licenses
• DEA
• Board certification
• BLS certification
• Certificates of insurance
• Driver’s license
• Professional headshot
• CME records (last 2 years)
• Immunization records
• NPDB report (within 30 days)
• References
