Requirements

• Board Certified or Truly Board Eligible in Gastroenterology — must become certified within 5 years
• Iowa medical license - Required
• DEA - Required
• BLS certification - Required
• Clean malpractice and license history - Preferred
• Recent Self-Query NPDB within 30 days - Required at presentation
• Visa requirements must be confirmed at submission
