Family Medicine Physician Opening in Portage, IN
Located in Portage, IN; Chicago 50m
Overview
* The health center is seeking a Family Medicine Physician for its primary care clinics (full time)
* Provide the full scope of primary care for patients of all ages within the Patient Centered Medical Home (PCMH) model
* Diagnosis, treatment and coordination of care, plus preventive and health maintenance services
* Work collaboratively with the patient care team, including nursing, behavioral health and social services staff
* Perform minor surgical procedures and arrange referrals for services not offered at the health center
* Must meet the health center's credentialing requirements, as well as the credentialing guidelines of affiliated hospitals in Lake and Porter County
* Schedule may include day, evening and weekend shifts
Qualifications
* MD or DO, graduate of an accredited Medicine program
* Indiana physician license, Indiana CSR license and DEA license
* Two years of experience in a healthcare setting preferred
