Overview:
* Join an established Primary Care practice within a community-focused healthcare organization
* Provide comprehensive primary care services with an emphasis on patient-centered treatment
* Assess, diagnose and manage patients with a variety of acute and chronic medical conditions
* Provide holistic care and ongoing management of an assigned patient population
* Exercise independent clinical judgment within the Nurse Practitioner scope of practice
* Practice under physician supervision as outlined in the position
* Collaborate with physicians, nursing staff and other healthcare professionals to coordinate patient care
* Utilize established clinical standards, guidelines and evidence-based protocols
* Participate in quality improvement initiatives to enhance patient outcomes
* Provide guidance and mentorship to clinical team members as appropriate
* Maintain a commitment to continuing education and professional development
* 3–5 years of progressive clinical experience involving primary care patient management required
