Overview:
* Join an established Primary Care practice within a nonprofit Federally Qualified Health Center
* Provide comprehensive outpatient primary care services to patients of all ages
* Full-time schedule consisting of 32 hours of direct patient care and 8 hours of administrative time per week
* Manage a diverse patient population, including patients with complex medical conditions
* Practice independently within a collaborative, team-based clinical environment
* Integrated behavioral health professionals and care coordinators available within each practice
* Access to dental services, community health resources and a growing Lifestyle Medicine program
* Work alongside physicians, advanced practice providers, clinical staff and healthcare leadership
* Provide evidence-based, patient-centered care emphasizing shared decision-making

* Practice within a certified Patient-Centered Medical Home
